This is for children K - 8th Grade. Please fill out all the information. There will be a picnic on Thursday, August 12th at Noon for the parents and children.
PLEASE NOTE: You must fill out the form for each child attending. For example - If you have 2 children attending, you must fill the form out twice - do not use the add person tab. This is very important!
